Every family must complete the PS29 Family Information Form for each child that is attending PS29.

 

There are unique form links for each grade, so please use the appropriate link below for each child that is attending PS29.

 

This form streamlines our processes and replaces the pile of paperwork that used to be sent home on the first day of school, including the blue emergency card, photo release form, walking trip permission slip, health information, dismissal and pick-up permissions and more. This is a REQUIRED form to help us gather the critical information we need about your child and family.

PreK and Kindergarten families completed a similar, but much shorter, "Registration Info Form" when you registered in the spring. We still need you to complete the "Family Info Form" below NOW so that we have all of the updated information we need for your child and family.   

 

Please be sure to take care in filling this form out accurately and promptly, as much of this information is needed on day 1 of school. BE MINDFUL of typos in your email addresses and phone numbers! This is a common issue!
